Job Description

The Care Coordination Placement Specialist assists in the coordination of post-acute care for El Camino Hospital patients. The coordinator works in conjunction with the health care team to assure timely access to post-acute care, maintaining continuity of care and optimizing patient well-being. The coordinator will help participants access appropriate community resources and negotiate transitions in their health care.

Key Responsibilities:

1. Process referrals for post-acute services

2. Schedule follow-up doctor's appointment

3. Perform discharge follow up phone calls

4. Conduct post-acute survey

Qualifications

Possesses the education and/or experience equivalent to that acquired in an accredited school of Vocational Nursing or related healthcare field. Bachelor's degree preferred

Three (3) years' experience working in post-acute services preferred

Excellent organizational, analytical and complex problem solving skills

Knowledge regarding aging issues, dementia care services, knowledge of community resources, and long-term care models

Ability to read, write and verbally communicate effectively

Excellent listening, interpersonal and customer service skills

Excellent organizational skills and ability to manage multiple tasks and timelines

Ability to work independently and to use sound judgment

Ability to establish and maintain effective, collaborative relationships with colleagues, providers, community agencies and staff

Intermediate computer application skills, including MS Windows, Word, Excel and Outlook

License/Certification/Registration Requirements

Licensed Vocational Nurse (LVN ) license for California

Current BLS certification

Salary Range:

$42.56 - $54.97 USD Hourly
