Job Description:

Central State Hospital is seeking a full-time Principal Case Manager.This position will primarily work Monday - Friday, 8 am-4:30 pm.
  • Coordinating treatment plans for individual patients, including: evaluation, consultation, education and follow-up services.

  • Identifying services available in the community that could potentially benefit patients, arranges for delivery of those services, and assists patients in gaining access to services, including scheduling appointments.

  • Conducting psycho-social evaluation upon admission. Providing psycho-social education to patients or their family members to increase their understanding of the patient's illness.

  • Participating as an inter-disciplinary team member on the unit as it relates to treatment planning, treatment reviews and discharge planning.

Required Qualifications:
  • A Master's degree in social work or a related field

  • Two years of experience

  • Basic knowledge of mental and physical characteristics of persons with serious mental illness;

  • Good communication and interpersonal skills;

  • Ability to manage case load with rapid patient turnover.
