This position is open to the public and all current City employees covered under the City's collective bargaining agreement with ILLINOIS NURSES ASSOCIATION - BARGAINING UNIT 37. Only employees in City job titles in this bargaining unit are eligible to bid.

Mission: Chicago Department of Public Health (CDPH) works with communities and partners to create an equitable, safe, resilient, and healthy Chicago.
Under the general supervision of a Physician, the class provides primary health care to patients within an area of specialty; and performs related duties as required
ESSENTIAL DUTIES
* Provides comprehensive sexual health care services to diverse patients aged 12 and older with known or suspected STI's or at risk for STI's
* Participates as a member of an interdisciplinary health care team, working in collaboration with medical, nursing, and administrative staff and disease intervention professionals with at least three (3) years of experience
* Performs patient history and physical examinations, reviews medical records, orders and analyzes laboratory and diagnostic tests, provides sexual health screening and health promotion, diagnoses illnesses and injuries, develops treatment plans, administers treatment and immunizations, and prescribes medications
* Documents findings and plans, places orders, and performs billing functions using an electronic medical record system
* Initiates referrals of patients to specialists as needed
* Collaborates with Supervising Physician to ensure quality of care
* Educates patients on preventative health interventions, including HIV pre-exposure prophylaxis, lifestyle modifications, and risk reduction measures
* Contributes to the development of goals and objectives relative to the program
* Participates in meetings and committees as well as community and clinic education programs
* Participates in continuing education programs to maintain nursing certification
* May supervise students, residents, and other trainees assigned to the program

Location: Various City-wide locations: 4909 W Division | 2849 N Clark | 200 E 115th St
Days: Monday - Friday
Hours: 8:00am - 4:00pm Monday, Wednesday, and Friday | 9:00am 5:00pm Tuesday and Thursday

MINIMUM QUALIFICATIONS
* Graduation from an accredited college or university with a Master's degree in Nursing with certification as a Nurse Practitioner in a specialized area is required.
Licensure, Certification, or other Qualifications
* Must be licensed to practice as a Registered Nurse and an Advanced Practice Nurse
* Must possess Individual Practice Authority (IPA) in order to work as a clinician in the clinic
* Applicants must possess a valid State of Illinois Registered Nurse license at the time of employment
SELECTION REQUIREMENTS
This position requires applicants to complete an interview which will include a skills assessment as part of the interview. The interviewed candidate(s) possessing the qualifications best suited to fulfill the responsibilities of the position, based on the oral and written parts of the interview will be selected.
Preference will be given to candidates possessing the following:
* Previous experience providing comprehensive sexual health care (e.g., STI and HIV preventative care) to diverse patient populations in medically underserved communities
* Considerable knowledge of best practices surrounding STI and HIV preventing, including testing and treatment recommendations
* Previous experience working in a multidisciplinary healthcare team
* Previous experience using electronic medical records system
