Responsibilities

  1. Oversees daily clinical and field operations related to in-home care delivery of HRC patients for the assigned market, including direct supervision and in-home shadow visits with the acute care RN team.
  2. Provides daily clinical and supervisory support for the assigned shift, including weekend rotation.
  3. Provides information to leadership necessary to generate clinical reports.
  4. Generates relevant clinical, and quality reports and provides/presents for market updates on a regular basis.
  5. Develops ACRN assignment/schedule in conjunction with ACCM and partnering supervisor as needed for all in-home care delivery of clinical staff and network providers (PT, OT, etc.)
  6. Works closely with the ACCM, RCCs, and HRC leader to facilitate communication and coordination between all members of the care team.
  7. Supervises ACRN clinical work and recovery course including daily clinical documentation and chart review and home shadowing visits.
  8. Guides ACRNs to promote effective performance and delivery of quality in-home care services through effective onboarding, education, mentoring and/or remediating clinical skills/processes/critical thinking,
  9. Assists ACRN team members in establishing immediate and long-term therapeutic goals, in setting priorities, and in developing the care plan.
  10. Ensures in-home clinical staff validate the accuracy of the admission health and home assessment data collected prior to HRC admission.
  11. Ensures effective communication of discharge information to other clinical departments or members of the Care Team including the Market RCCs, VCU RCCs, PCP, and health plan case manager, as needed.
  12. Participates in the market screening and interview process of new employees and makes recommendations for the employment of individuals.
  13. Provides the clinical orientation process through didactic, side-by-side, and in-home shadowing methods.
  14. Complies with accepted professional standards and principles and ensures staff practice within their state's scope of practice.
  15. Assists in the formulating, revising, implementing, and evaluating organization policies, procedures, goals, and objectives, both short and long range.
  16. Actively participates in performance improvement activities.
  17. Acts as an ACRN, carrying a reduced-level patient census.
  18. Complies with policies, procedures and regulatory mandates including but not limited to abiding to the terms of the Amedisys Compliance Program.
  19. Performs other related duties as assigned.

Qualifications

Required:
  • Associate of Science in Nursing degree.
  • Current, active RN state licensure.
  • Two (2+) years ofacute care nursing experience.
  • One (1+) year of experience managing direct reports.
Preferred:
  • Bachelor of Science in Nursing degree.
  • Three (3+) months of experience as an acute care RN providing in-home care.
