Job Description
  1. Serves as first point of contact for all non-urgent incoming pharmacy communications through the call center or medical records.
  2. Leverage out-patient pharmacy technician expertise to appropriately triage incoming medication refill requests, medication prior authorization requests, prescription clarifications and medication supply shortage issues.  
  3. Responsible for coordinating and tracking the steps of the medication prior authorization process.
  4. Interfacing with appropriate SSCHC team members to solve logistical medication inquiries and prescription insurance related issues.
  5. Coordinate with Medication Access Specialists to expeditiously maximize patient access to medications.
  6. Coordinate with appropriate team members to maintain compliance of in-clinic medication stock to all State and Federal regulations as well as Joint Commission standards and SSCHC policy and procedures.
  7. Review all external pharmacy communications sent from call center or medical records to determine and execute action steps to appropriately triage and respond to the pharmacy communication.
  8. Document in medical record pertinent steps taken to address medication/pharmacy related communications and inquiries.
  9. Maintain communication with patients and clinical teams with regards to the status and completion of medication/pharmacy related inquiries.
  10. Coordinate and track steps in the medication prior authorization process including initiation PA process, communicating with appropriate clinical team member to obtain pertinent clinical information, collect signatures, submit PA’s, follow up on outstanding PA’s, communicate approvals or denials with clinical team and coordinate communication with the patient.
  11. Coordinate with Medication Access Specialists to ensure eligible patients are enrolled in cost saving programs.
  12. Serve as a liaison between patient financial services, clinical team, patient, and external pharmacy when assisting patient through prescription insurance related inquiries.
  13. Work with clinical pharmacist team to assist with patient outreach and communication when necessary.
  14. Coordinate with clinical team members, medical assistant Supervisors, Director of Nursing, clinic managers, clinical pharmacists, medical directors, epic analysts and supply chain staff to maintain medication dispensaries including: monitoring of expiration dates, rotation of stock, appropriate labeling, sequestering and disposal a medication compliant with all laws, regulations and policies monitoring and responding to medication recalls and maintaining policies and procedures related to medication dispensary activities.  
  15. Maintain compliance with all local, state, and federal laws regulating your specific practice.
  16. Promote and maintain compliance with all Sixteenth Street Clinic Policies and Procedures.
  17. Other duties as assigned.

QUALIFICATIONS:
 
  1. Proven high level written and oral communication skills.
  2. Self-motivated and independent, able to work without direct supervision.
  3. Strong background and understanding of the prescription prior authorization process and prescription insurance plans.
  4. Ability to work well within a multidisciplinary team.
