We are seeking a Case Manager for our Pathway Prevention Program to join our team in our Wilmington, DE location.

Position Details

The position coordinates case management and linkages to community programs and services. This role coaches/counsels' individuals/families within their caseload and provides crisis intervention.

Case sessions are primarily delivered in the community (client homes, schools, etc.), case management can be in an office environment or community. May at times be required to work evenings and/or weekends to meet the needs of the clients. This position may require on-call coverage.

Earn: $43k+ per year

Requirements

The ideal candidate will possess the following qualifications:

  • Bachelor's Degree required, preference is in psychology, sociology, social work.
  • 1-3 years of experience working with children.
