Job Summary:

The Service Coordinator is responsible for coordinating the services for youth enrolled in the Community Youth Development program. Program services are available to participants according to their social, community and school needs, which is disclosed upon intake. The Service Coordinators duties include the ability to facilitate program components on a weekly basis, specifically the YLD and Parent involvement components. Service Coordinator ensures youth have access to all program components. Program services include Youth Leadership Development Sessions, Tutoring, Mentoring, and Parents Groups. The Service Coordinator serves as a family liaison, community outreach worker, and group facilitator.

Essential Functions:
    Conduct initial intake with client and family/guardian.
  • Ensure the timely completion of all required documentation (ie: Intake, Assessments and Individual Service Plans, etc.) in accordance with Southwest Key Programs (SWK) policy.
  • Ensure all daily documentation is completed and entered into PIERS & SWK database in a timely and efficient manner.
  • Maintain physical and web-based program participant files.
  • Establish and maintain a healthy and professional relationship with clients and families.
  • Ensure clients participate or are offered all components of the program.
  • Provide crisis intervention for youth and their families.
  • Provide monthly home visits with youth and families.
  • Provide advocacy, referrals and linkages to program participants and their families.
  • Recruit and retain participants in programming in coordination with other program staff.
  • Responsible for scheduling, implementing outreach recruitment efforts.
  • Facilitate and/or co-facilitate group counseling and recreation sessions for youth, as well as parent participants.
  • Prepare written reports and presentations related program services and deliver to appropriate audiences, as needed.
  • Collaborate with stakeholders in creating and maintaining program services at various locations.
  • Collaborate with school administration and community agencies for service delivery and outreach opportunities.
  • Able to react to change productively and handle other essential tasks as assigned in order to meet needs of the program.
Other Functions:
  • Attend and participate in all required professional trainings and workshops.
  • Maintain open and consistent communication with Program Director, Community Agencies, School Administration and other team members.
  • Transport clients to program activities, as needed.

  • Demonstrated ability to use data analysis for problem solving.
  • Strong background in working in communities of high-need.
Qualifications and Requirements:
  • Bachelor's degree in a social service or related field with two (2) years' experience working with at risk youth and their families; may include part time, volunteer or internship experience.
  • Possess good verbal and writing skills.
  • Clear background check from appropriate entity.
  • Must possess a valid state driver's license, registration and maintain state mandated car insurance and be eligible to drive to facilitate program services as required.
  • Works well independently, as well as an effective team member.
  • Working knowledge of Microsoft 365.
  • Well organized, meticulous and thorough in work habits.
  • Required to work a flexible schedule to facilitate program services, occasional weekend work is required. The position may require more work hours than the normal eight-hour workday.
  • Bilingual English/Spanish.
Physical Demands:

Must be able to obtain Crisis Prevention Institute (CPI) certification and First Aid Certification (CPR). Must be able to supervise clients indoors and outdoors as necessary. Bending, stooping and lifting up to 15 lbs required to complete daily tasks. Job requires extended periods of sitting; use of various office equipment.

Work Environment:

Ability to work a flexible work schedule. Work schedules are subject to variation from time to time depending on the needs of the department, facility or program.
