Now Hiring: Join Central City Concern as a Client Access Services Navigator - Supportive Housing

Location: Central City Concern - Old Town Recovery Center (Downtown Portland)

Schedule: Monday - Friday (8:00am - 5:00pm)

SUMMARY: The Client Access Services Navigator position is a member of the multi-disciplinary team that assumes responsibility and accountability for the management of resources through interdisciplinary collaboration to achieve optimal client outcomes. This role engages directly with people to understand their needs and help facilitate connections across the full continuum of CCC's services including Health Services, Housing, and Employment services.

This work happens directly in the field with clients to identify programs, resources, and services that will best help meet their needs while removing obstacles to accessing programs, resources, and services. This position has access to mental health billing and service records.

MINIMUM QUALIFICATIONS (REQUIRED)

For formal consideration for the Client Access Services Navigator position, please ensure your professional/working experience meets the following requirements:

  1. Required: High school diploma or GED.
  2. Required: Minimum of two years of work experience in a relevant field such as human services, community health, social services or related field of these or combination of work and education.
  3. Required: Must have experience in case management, customer service or advocating for clients.
  4. Required: Valid driver's license.
  5. Required: Computer literacy.

PREFERRED QUALIFICATIONS (I.e. "Nice-to-Haves," but not required)

  1. Bachelor's Degree preferred in medical field, mental health, or social sciences or related field.
  2. Bilingual in English and Spanish.

Essential duties and responsibilities:

  1. Conduct Social Determinants of Health based interviews with clients in Recovery Supportive Housing to assess and identify health disparities and inequities (I.E food insecurity, housing insecurity, transportation insecurity).
  2. Assist clients in engaging with health and social services by performing intakes and scheduling appointments.
  3. Provide transportation when necessary for patients in CCC vehicle, taxi, or public transportation to and from program related appointments, interviews, and hospitals/clinics.
  4. Provide enrollment assistance (including but not limited to completing coverage applications, gathering required documentation and troubleshooting the enrollment process) for uninsured adults to access subsidized, low-cost and free health insurance programs through the health insurance marketplace and Medicaid
  5. Provide structured patient education on health coverage, engage in follow-up conversations and offer renewal assistance for enrolled individuals.
  6. Help people to seek opportunities for further treatment, care, and services within Central City Concern and the community.
  7. Participate in the development of health and recovery goals with people. Support people to reduce barriers and achieve goals.
  8. Bridge community, cultural, educational, and other barriers to encourage self-care and participation in CCC Recovery Supportive housing and health service offerings.
  9. Complete and maintain accuracy of all required documentation including electronic documentation in a timely manner and in accordance with regulations and office standards.
  10. Conduct compliance assessments and internal audits. Analyze results and provide formal and informal reports of findings, including recommendations for improvement. Effectively communicate findings to manager, colleagues, and internal and external stakeholders and customers, and assist with related corrective action as needed.
  11. Adhere to all state and federal privacy regulations, including HIPAA and 42 CFR Part 2, and to CCC policies and agreements regarding confidentiality, privacy, and security. Support compliance with all privacy and security requirements pursuant to community partners' and outside providers' patient confidentiality agreements, including privacy and security requirements for EMR access. This includes immediately reporting any breach of protected health information or personal identification information of any person receiving CCC services by CCC or an outside provider to the CCC Compliance Department, as well as to your supervisor or their designee.
  12. Attend all mandatory CCC trainings in a timely manner.
  13. Participate in compliance, quality assurance and quality improvement activities as directed.

DESCRIPTION OF OTHER DUTIES:

  1. Become certified State of Oregon Enrollment Assister and assist individuals in enrolling for healthcare coverage as directed by supervisor.
  2. Perform other duties as assigned.

  5. Ability to develop relationships and work courteously and effectively communicate with the general public, patients, medical personnel, corrections personnel, police, merchants and co-workers.
  6. Ability to maintain accurate records and necessary paperwork.
  7. Ability to maintain strict standards of confidentiality with regard to patient information.
  8. Ability to transport patients in company vehicle.
  9. Ability to handle emergency/violent situations with skill, confidence and tact.
  10. Ability to work unsupervised for long periods of the day.
  11. Ability to maintain appropriate professional boundaries.
  12. Ability to perform simple mathematical computations.
  13. Ability to manage time, prioritize, and meet deadlines.
  14. Excellent organizational skills.
