Central City Concern is looking to hire On-Call Primary Care Providers (NP, PA, MD, DO)

*A DATA2000 waiver for buprenorphine prescribing is not required, but is strongly preferred.

Schedule: On-call shifts include Monday - Friday shifts in primary care and Saturday shifts in Quick Care. In both settings, the Blackburn Center offers Office Based Opioid Treatment (OBOT) services.

As an on-call primary care provider, you will use your strong background in mental health, addiction medicine, and health care to offer integrated, client centered, trauma informed care to homeless and underserved populations. You will help support patients in achieving their housing, health, and resiliency goals. Using your background with the client population, you will collaborate with staff to determine health histories and continue to formulate this history with appropriate documentation and record keeping. Your ability to consider impacts of structural racism and oppression on individual client outcomes, inform your use of de-escalation strategies and your daily practice.

MINIMUM QUALIFICATIONS: NP/PA
1. Graduate of an accredited Nurse Practitioner or Physician Assistant program.
2. National certification and unrestricted license or eligibility for license to practice in Oregon.

MINIMUM QUALIFICATIONS: MD/DO
1. Board eligibility or certification in Internal Medicine or Family Practice.

MINIMUM QUALIFICATIONS: ALL APPLICANTS

  • Experience with Hepatitis C primary care-based treatment preferred, or provider will obtain training within 30 days of employment.
  • Must meet CCC privileging requirements as required by FTCA.
  • Current DEA Certification required or ability to obtain within 30 days of employment.
  • Current CPR certification or ability to obtain within 30 days of employment.
  • Available to work during standard clinic hours, occasional evenings until 8 pm, and Saturdays 9 am -1 pm 1 - 2 / month.
  • If a recovering chemically dependent person, must be in an on-going program of recovery with two (2) years of demonstrated level of sobriety.
  • Must pass a pre-employment drug screen and DHS background check.
  • Adhere to all state and federal privacy regulations, including HIPAA and 42 CFR Part 2, and to CCC policies and agreements regarding confidentiality, privacy, and security. Support compliance with all privacy and security requirements pursuant to community partners' and outside providers' patient confidentiality agreements, including privacy and security requirements for EMR access. This includes immediately reporting any breach of protected health information or personal identification information of any person receiving CCC services by CCC or an outside provider to the CCC Compliance Department, as well as to your supervisor or their designee.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

  • Perform physical exams and order, interpret and evaluate diagnostic tests to identify and assess patients' clinical problems.
  • Provide provider services in Office Based Opioid Treatment program (If provider does not have a DATA2000 waiver, conduct office visits and communicate with OBOT team about ongoing medication needs. If has a DATA2000 waiver, prescribe buprenorphine).
  • Conduct initial care for emergency situations such as illness or injury and initiate arrangements for continuing care.
  • Prescribe medication or other forms of treatment as necessary.
  • Assess need for and perform clinical procedures, including joint injections, incision and drainage of abscesses, skin biopsies, and toenail removal. May be able to perform other procedures pending privileging by Blackburn Associate Medical Director.
  • Proactively identify opportunities for improvement within Blackburn Center workflows and logistics. Participate in review of Blackburn Key Performance Indicators; review and contribute to continuous improvement opportunities and plans.
  • Build understanding of and appreciation for diversity among Blackburn Center clients and support their connection to culturally responsive and culturally specific services as requested.
  • Comfort working in an independent clinical practice with on and off-site support from medical and administrative supervisors.
  • Attend all mandatory CCC trainings in a timely manner.
