COMPANY OVERVIEW:

Community Solutions Inc. (CSI) is a nonprofit organization that promotes the responsible citizenship, accountability, and well-being of individuals and families involved, or at risk of involvement, in the child welfare, juvenile justice, or criminal justice system. Since CSIs founding in 1962, it has been a pioneer in the community-based service. We have been delivering supervision, treatment, and comprehensive individualized services to help clients interact more effectively in their communities and become productive citizens. Using this plan we built the first ever halfway house in New England which has now been replicated and grown to almost 50 programs in 9 states. For over 50 years, CSI has been building upon its successful foundations and has been expanding its services to serve thousands of adults, youth, and families each year.

CSI understands clients and staff have been impacted by trauma. CSI is becoming a trauma-informed organization by hiring individuals with existing experience in trauma-informed practices as well as supporting ongoing staff development with trauma-informed practices. This will include learning to recognize symptoms of trauma and how to respond, through training starting at orientation and throughout their career at CSI. CSI is committed to systematically evaluating the integration of the trauma-informed values of safety, trustworthiness, choice, collaboration, and empowerment at all levels of the organization's policies, procedures, and structures to guard against the possibility of re-traumatization.

PROGRAM OVERVIEW:

Community Solutions is seeking a Case Manager to join our team at the Aileen O'Connor House in Hartford, CT. General case management duties including intake assessment, individual and group counseling, case record management, discharge preparation and aftercare services

Schedule: Monday/Wednesday/Friday 9 am - 5 pm, Tuesday/Thursday 1 pm - 9 pm.


Essential Functions include but are not limited to:

  • Check daily for newly assigned clients.
  • Complete assessments within 3 business days of intake,
  • Facilitate feedback and creation of ISP within 5 business days of intake.
  • DOC case management sessions are to be held based on the clients assessment scores.
  • DOC clients: Submit monthly progress reports to your supervisor 4 calendar days before due date.
  • BOP case management sessions are to be held at least once per week (every 7 calendar days) for their first 6 weeks.
  • BOP Clients: Submit weekly progress reports for the first (6) six weeks and bi-weekly progress reports thereafter.
  • Home confinement clients: Collect and review phone records on a monthly basis to ensure compliance with the SOW.
  • Complete and submit discharge plan for BOP and DOC client 60 days before scheduled release date.
  • Prepare, plan and facilitate Transitional Skills (Wilmington, WH, HH and Chase) and R&R group-sessions
  • Record all sessions with clients and follow procedure to upload these recordings for quality assurance review.
  • Actively participate in quality assurance review session and apply new strategies and techniques presented.
  • Actively participate in PRT meetings by presenting new clients assessments and ISP, updates on clients nearing discharge, provide and elicit peer support for client issues.
  • Make recommendations to the supervisor for corrective action as required and monitor on an ongoing basis.
  • Maintain client files and ensure compliance with Agency policy regarding confidential information.
  • Assist clients with obtaining birth certificate and social security card.
  • Home Confinement clients (Wilmington, WH, HH and Chase only) you may be required to
    • Conduct CM sessions at the persons home confinement site
    • Secure urine specimen and Breathalyzer test at the HC site
    • Conduct a search of the clients vehicle while at the HC site
  • On a monthly basis collect and make copies of the clients bank records to ensure it balances with our financial records.
  • Complete incident reports as needed and submit to PD prior to leaving shift.
  • Register for and attend all required trainings and boosters including but not limited to MI, Case Management and Assessments.
  • Secure urines from same sex clients and complete all required paperwork
  • Monitor case load to ensure all urines and Breathalyzer tests are randomly requested and secured as required per funding source.
  • Conduct weekly audits of all client medications to ensure clients are med compliant
  • Conduct employment, job search and home confinement spot checks
  • Conduct room searches, common area searches and perimeter searches
  • Conduct pat/pocket searches, facility rounds, and house counts as needed.
  • Assist with serving of meals when assigned
  • Serve as a role model to clients and staff modeling pro-social behaviors.


QUALIFICTIONS:

  • Bachelors degree in a social science field and one year experience in case management or similar field. Education may be substituted with experience on a year for year basis.
  • For FBOP Sites Education cannot be replaced with experience and at least a Bachelors degree is required.
  • Excellent oral, written and interpersonal skills desired.
  • Ability to engage and develop collaborations with community agencies.
  • Previous assessment experience preferred
  • Experience with Microsoft Office
