The Center for Development and Disability (CDD), established in 1990, is New Mexico's University Center for Excellence in Developmental Disabilities Education, Research and Service. These centers, authorized by the Developmental Disabilities Assistance and Bill of Rights Act, build the capacities of states and communities to respond to the needs of individuals with developmental disabilities and their families.
Join our team! The UNM/CDD Family and Community Partnerships Division is seeking to hire experienced professionals to provide education and support coordination to individuals receiving Medicaid Waiver services in New Mexico. The individual hired for this position will help participants navigate the Mi Via and Supports Waiver systems. The positions will provide assistance and guidance with eligibility, certification, pre-planning, service and support plan development, and budget development as part of their assigned caseload.
The role includes conducting monthly visits to see participants in their homes and community, providing education and training, and maintaining electronic clinical files in compliance with state guidance. The successful candidate will be working with a dynamic team of professionals to meet the needs of individuals and their families throughout New Mexico.
The positions require regular statewide travel. A remote work arrangement will be considered for candidates outside of the Albuquerque Metro Region.

Bachelor's degree; at least 3 years of experience directly related to the duties and responsibilities specified.
Completed degree(s) from an accredited institution that are above the minimum education requirement may be substituted for experience on a year for year basis.
