Part-Time (20 hrs/week)Job Description:
Responsible for providing case management/social work services at Bellin Oconto hospital in the Northern Region. Working on a team to assist with guiding our patients to have the best possible health outcomes by eliminating barriers to health care, helping with psychosocial needs, addressing financial concerns and connecting to resources in the community. Also assist with evaluating and eliminating social determinants of health barriers and assuring smooth care transitions throughout the continuum of care.
Wisconsin Social Work license required.
Bachelor's degree in Social Work required, Master's degree in Social Work preferred.
Skills in assessing, planning, and managing patient care in an acute care hospital or other health care facility recommended; background in care management, utilization review and discharge planning is beneficial; knowledge of available community resources to assure the continuum of care is beneficial.Why Bellin Health:
