Full-Time 0.80 FTE (32 hours/week), Days (8-5)/No Nights/Monday-Friday/No Weekends/No Calls/No Holidays

Responsible for nursing care under the direct supervision of the Provider/Registered Nurse. Records nursing care given and reports to appropriate person with changes in patient's condition; consults with RN/Provider when uncertain about nursing or medical act; provides basic health instructions and reinforces teaching by other health team members.

Appropriate state LPN license required. Valid Wisconsin driver's license required for Screen Team and Employer Clinics as of 8/28/2014.
