• 0.90 FTE (36 hours/week), 8:00AM - 5:00PM Monday thru Friday/ No Call/ No Weekend / No Holiday

Valuable team member of our team-based care model that provides direct nursing care under the supervision of a registered nurse/provider treating patients of all ages. This important role involves providing advanced patient care, observe patients' total health and wellbeing, assisting providers, and communicating instructions to patients regarding home medications, home-based care, and preventative lifestyle changes.

Responsible for nursing care under the direct supervision of the Provider/Registered Nurse. Records nursing care given and reports to appropriate person with changes in patient's condition; consults with RN/Provider when uncertain about nursing or medical act; provides basic health instructions and reinforces teaching by other health team members.

WI LPN license -OR- graduate of an accredited Medical Assistant program. If LPN, license required within 3 months of start date. If Medical Assistant, AAMA certified within 3 months of start date.
