Full-Time 1.00 FTE (40 hours/week), Mon-Fri 8am-4:30pm.
Interested in a role that provides an independent work environment while making a positive impact on high risk patient populations? The Care Coordinator Role may be for you. We are looking to add either a registered nurse (RN) or a licensed practical nurse (LPN) to join our team!
The Care Coordinator works within their scope and acts as the primary mediator between high risk patients and involved healthcare entities to achieve improved patient outcomes, compliance, and satisfaction. They work collaboratively with the multidisciplinary care team to identify high risk patients requiring enhanced healthcare navigation and education, encompassing coordination of care across the continuum. Community involvement and collaboration is encouraged. The care coordinator develops and implements patient specific goals to reduce care gaps and barriers for their managed patient panel. The length of time a care coordinator would follow a patient is determined by patient needs and attainment of mutual goals. The care coordinator functions within the full scope of nursing practice adhering to the WI Nurse Practice Act, Administrative Code and/or appropriate nursing standards governing nursing in the state of practice, and Bellin Health Policies and Procedures.Qualifications:
WI RN or LPN license required and active WI driver's license.
Two years nursing experience required in clinical or acute care setting. In addition, experience in care/disease management, health coaching, utilization review, and/or discharge planning preferred. Demonstrated ability of adhering to clinical and regulatory standards, program development, managing department protocols and processes, and managing patient flow required.
