JOB RESPONSIBILITIES:
Provide case management to all patients of Sixteenth Street Community Health Centers beginning at onset of prenatal care until 8 weeks postpartum. Responsibilities include but are not limited to:
Performance of risk assessments at the beginning of each pregnancy and as necessary
Refer patients to Sixteenth Street or community agencies/specialists as needed
Health education
Postpartum home visits as appropriate
Provide breastfeeding education and support
Supervision of ancillary staff
Provide parents with newborn education and ensure appropriate scheduling of follow up care with pediatrician
Coordinate care of pregnant patients with the OB providers
Comply with State of Wisconsin requirements for perinatal care coordination (PNCC) and OB Medical Home (OBMH) as outlined in the State of Wisconsin handbooks and the Sixteenth Street Policies and Procedures
Maintain appropriate patient records for the PNCC and OBMH programs. This may involve some oversight of the Prenatal Care Coordinators
Maintain the Sixteenth Street OBMH Master List and communicate effectively with the Finance Department about the patients in the OBMH program
Maintain the HMO OB Registry ensuring that it is kept up to date
Comply with SSCHC TB Control Plan including PPD testing
Maintain BLS certification
Perform other duties as assigned
QUALIFICATIONS:
Registered nurse or Health Educator with a Master’s in Health Education and 2 years’ experience in community health services or Social Worker with a Bachelor’s degree and minimum of 2 years’ experience in a health care or family services program
Excellent oral and written communication skills
Computer and typing skills desirable
Ability to relate well to people from diverse ethnic and cultural backgrounds
