Job Description
Join our team who is committed to the delivery of the highest quality health care service.  We are seeking a Department of Environmental Health & Community Wellness Manager that is self-motivated, energetic, and a take charge individual.

Job Responsibilities:
  1. Provide strategic vison for Community Health within Milwaukee’s southside (SSCHC service area)
  2. Develop and maintain a comprehensive plan to address the health of the community within the service area of SSCHC, including addressing the significant social drivers of health and fostering health equity.
  3. Perform a locally based (within service-area, if possible) needs assessment at a minimum of every three years, according to HRSA standards and guidelines, to inform the health plan for our service area
  4. Identify health needs within the community and gaps in community-based resources (within and outside SSCHC) that address those needs.
  5. Identify programming to fill those gaps that is community-facing and complements the work done within SSCHC clinics and site-based programs.
  6. Develop, implement, and oversee SSCHC programs within the Community Health Department
  7. Lead an interdisciplinary team in creating and delivering programs and services that address health and social needs of patients and community members.
  8. Participate as an active, contributing member of the Sixteenth Street Leadership Team:
  1. Attend regular LT meetings, periodic retreats and team building activities and meetings of the Board of Directors as requested.
  2. Participate in strategic planning activities and help align agency strategic initiatives to patient, client, and community needs.
  3. Provide leadership and management of operations that ensure a strong and stable agency financial position, including participation in revenue generation, grant seeking and fund development activities.
  4. Serve as a member of the Quality Improvement Committee.
  1. Provide strategic guidance, direction, and ongoing support to all programs within Community Health Department
  1. Set strategy for each program and assist with operational planning to meet the strategy.
  2. Establish and maintain alignment of Community Health programs to the needs of Sixteenth Street patients, clients, and the broader community.
  3. Create and oversee structures and mechanisms that ensure effective communication and proper planning, maintaining, tracking, evaluating, and reporting of Community Health activities.
  4. Ensure ample financial and human resources to achieve and maintain program effectiveness.
  5. Oversee staff in the management of, and application for, numerous grants both public and private ensuring appropriate fiscal and outcome management and assisting with compliance oversight.
  1. Where appropriate, integrate the operations of Community Health programs with clinical operations other services delivered by Sixteenth Street.
  2. Oversee the supervision of managers and staff to ensure high quality program delivery and consistency with the agency employee handbook and other relevant policies and procedures.
  3. Approve and oversee annual department and program budgets as part of the agency fiscal year budget process.
  4. Initiate strong relationship with grantors and advancement team to identify partnership opportunities and to ensure a secure financial plan to support Department endeavors.
  5. Promote and maintain positive relationships with external project partners, collaborators, government representatives and other stakeholders whose support of Sixteenth Street are critical to future success.
  6. Represent SSCHC within community health coalitions and on advisory boards as appropriate, and at community functions and conferences at the local, state, and national level.
  7. Comply with SSCHC TB Control Plan including PPD testing.
  8. Perform other duties as assigned by the President & Chief Executive Officer.

Qualifications:
  1. Advanced degree in a public health (DrPH preferable, MPH considered with extensive experience); consideration will be made for a suitable combination of previous academic and professional experience.
  2. Substantial experience in all aspects of community-oriented program development and management, including supervision of staff, grant development/reporting, monthly and annual budgeting, program analysis and evaluation.
  3. Ability to relate well to people of various ages and from diverse ethnic and cultural backgrounds.
  4. Excellent oral and written communication skills.
  5. Bilingual (Spanish/English) communication skills preferred.
  6. Ability to work independently and in a group setting.
  7. Proficiency with MS Office and experience with MS Access or similar database platforms.
  8. Valid Wisconsin driver’s license, use of an automobile and current automobile insurance.
