Job Description
Join our team who is committed to the delivery of the highest quality health care service. We are seeking a full-time CCS – Rehabilitative Support Specialist that is self-motivated, energetic, and a take charge individual.

General Statement of Duties:

The Comprehensive Community Services (CCS) Rehabilitative Support Specialist position serves as a vital member of the CCS team providing individual skill development in areas of home care, home maintenance, budgeting, wellness activities and enhancement services to support clients in how to reach their recovery goals. The Rehabilitative Support Specialist serves people at various stages of their recovery from mental health, substance use, and co-occurring needs by embracing the person's strengths to meet their identified needs.

Responsibilities:
  1. Role varies on each team and services are provided in the community to meet the needs identified in the Individual Recovery Plans of Care (RPOC). 
  2. Be an active member in client/consumer 6 month and annual review plans.
  3. Participate in staff training, read materials, program forms, and other program documents.
  4. Engage in ongoing staff training, coaching, skill development, and performance evaluation.
  5. Ensure clients are receiving quality services through supervision and consultation with CCS Administrator.
  6. Provide services in the community including people's homes and other community locations.
  7. Appropriately document all contact with the people being served and team members to adhere to program requirements.
  8. Responsible for accurate documentation and maintaining a case file as required by agency policy, government regulations, and applicable local, state, and federal codes, including DHS 36 for CCS program.
  9. Provide coverage for staff, as needed, for vacations and holidays when applicable.
