Job Description:
- Field-based RNs conduct clinical eligibility assessments for individuals seeking Medicaid eligibility who reside in community-based settings or nursing facilities state wide.
- Travel is required for this position.
- A background check for motor Vehicles is required as well as a personal vehicle.
- The nurse will be responsible for reviewing medical records, interviewing individuals, and completing a comprehensive standardized assessment tool within hospitals, nursing facilities, assisted living, senior housing, and private community homes.
- The nurse will also be responsible for providing counselling on service options.
- Training and mentoring is provided over 4-8 weeks.
- Training sessions may be virtual or in-person in the regional field office.
- Communication Skills.
- Info gathering & documentation.
- Interpret rules and follow established processes.
- license issued by the State of NJ.
- PPD or chest x-ray.
- Physical Report.
- Covid Test Report.
- Flu shot (Current Season).
- BLS/CPR.
