CCS Intake Care Coordinator (Level 2)

Job Description
We are seeking a full-time CCS Intake Care Coordinator (Level 2) that is self-motivated, energetic, and a take charge individual.

- Complete initial Comprehensive Assessment for intake process
- Complete initial Assessment Summary for the CCS intake process.
- Complete Recovery Plan of Care (RPOC) and invite assigned Care Coordinator who will take over the on-going care for the consumer in the CCS program.
- Develop, update, and conduct 6 month and annual reviews in the Electronic Health Record as needed for caseload outside intake duties.
- Participate in staff training, read materials, program forms, and other program documents.
- Engage in ongoing staff training, coaching, skill development, and performance evaluation.
- Ensure clients are receiving quality services and add to the initial plan if they come into the program with them in place prior to admission.
- Provide ongoing documentation for services rendered to the consumers/clients. 72-hour guideline for note entry and bill Medicaid appropriately for intake and care coordination services.
- Provide coverage for staff, as needed, for vacations and holidays when applicable.
- Other duties as assigned.
- Must possess a B.A. or B.S. degree in Social Work, Psychology, Nursing, or a related field with experience in human services and case management. Master’s degree preferred.
- Prior experience in leading or managing treatment teams in a human service program preferred
- Valid Wisconsin driver’s license and automobile with insurance required in good standing.
