Job Description
We are seeking a full-time We are seeking a full-time CCS Intake Care Coordinator (Level 2) that is self-motivated, energetic, and a take charge individual.
 
Job Responsibilities: 
.
  1. Review initial Comprehensive Assessment for intake process.
  2. Review initial Assessment Summary for the CCS intake process.
  3. Review and attend when available the Recovery Plan of Care (RPOC) with intake Care Coordinator and Care Coordinators Level 1 and Level 2 for the on-going care for the consumer in the CCS program.
  4. Develop, update, and conduct 6 month and annual reviews in the Electronic Health Record as needed for caseload outside of caseload duties.
  5. Participate in staff training, read materials, program forms, and other program documents.
  6. Engage in ongoing staff training, coaching, skill development, and performance evaluation.
  7. Ensure clients are receiving quality services and add to the initial plan if they come into the program with them in place prior to admission.
  8. Provide ongoing documentation for services rendered to the consumers/clients.  72-hour guideline for note entry and bill Medicaid appropriately for intake and care coordination services.
  9. Provide coverage for staff, as needed, for vacations and holidays when applicable.
  10. Other duties as assigned.
Qualifications:
 
  1. Must possess an LPC-IT or APSW degree from State of Wisconsin and be eligible for future full licensure to an LPC or LCSW respectively.
  2. Must be at or above 1,500 of the required 3,000 hours towards full licensure.
  3. Prior experience in leading or managing treatment teams in a human service program preferred.
  4. Valid Wisconsin driver’s license and automobile with insurance required in good standing.
  5. Pre-employment background check.
  6. Must possess a master’s degree in social work, Psychology, Nursing, or a related field with experience in human services and case management.
