Join our team who is committed to the delivery of the highest quality health care service. We are seeking a full-time Women's Wellness Team Leader that is self-motivated, energetic, and a take charge individual.
Responsibilities:
Oversee and manage all members of the Women’s Wellness Department which includes management of time and attendance, conducting performance evaluations, and being involved in disciplinary procedures with the assistance of the VP of Medical Affairs.
Schedule, design, and conduct department meetings. This includes organizing and tracking any mandatory trainings that need to be completed.
Organize the hiring, training, and onboarding for all new hires.
Conduct OB Case Management responsibilities which includes providing case management to all patients of Sixteenth Street Community Health Centers beginning at onset of prenatal care until 8 weeks postpartum. Responsibilities include but are not limited to:
Performance of risk assessments at the beginning of each pregnancy and as necessary
Refer patients to Sixteenth Street or community agencies/specialists as needed
Health education
Postpartum home visits as appropriate
Provide breastfeeding education and support
Supervision of ancillary staff
Provide parents with newborn education and ensure appropriate scheduling of follow up care with pediatrician
Coordinate care of pregnant patients with the OB providers
Oversee the State of Wisconsin perinatal care coordination (PNCC), Child Care Coordination (CCC), and OB Medical Home (OBMH) programs which includes insuring compliance with state guidelines as outlined in the State of Wisconsin handbooks and the Sixteenth Street Policies and Procedures.
Along with the Women’s Wellness team, maintain appropriate patient records for all programming.
Along with the Women’s Wellness team, maintain the Sixteenth Street OBMH Master List and communicate effectively with the Finance Department about the patients in the OBMH program.
Oversee the maintenance of the HMO OB Registry ensuring that it is kept up to date.
Work with the VP of Medical Affairs on all QI initiatives that the department is expected to participate in.
Assist the VP of Medical Affairs with any required information for grant reporting and for new grants being applied for.
Qualifications:
Registered nurse or Health Educator with a Master’s in Health Education and 2 years’ experience in community health services or Social Worker with a Bachelor’s degree and minimum of 2 years’ experience in a health care or family services program.
Excellent oral and written communication skills.
Computer and typing skills desirable.
Ability to relate well to people from diverse ethnic and cultural backgrounds.
