Job Description
Join our team and provide high quality assessment, intervention, and consultation within integrated primary care teams at 16th Street Community Health Center. Our Behavioral Health Consultants focus on providing rapid access to high quality and evidence-based interventions to address a broad range of mental/ behavioral health concerns. Ideal candidates enjoy a fast-paced work environment and are passionate about serving an underserved population.  Ideal candidates have experience in or are open to training in working in an integrated primary care setting, are comfortable providing treatment across the lifespan, and enjoy providing both in-person and telehealth services to increase access to care.


Responsibilities: 
  1. Adhere to the population-based, consultative role of the primary care behavioral health model.
  2. Adhere to the principle of the primary care provider as the “first customer” of the behavioral health consultation service.
  3. Assess the clinical (i.e., behavioral, and mental health) status of patients referred by primary care providers through brief consultative contacts.
  4. Assist primary health care providers in assessing mental health concerns and psychosocial problems.
  5. Assist the primary care team in adhering to and making decision related to evidence-based psychotropic medication algorithms.
  6. Work with primary care team to monitor and manage patients with chronic emotional and/or health problems efficiently and effectively.
  7. Work with primary care provider to refer cases to specialty mental health providers as appropriate.
  8. Assist in the detection of “at risk” patients and development of plans to prevent further psychological or physical deterioration.
  9. Assist in preventing relapse or morbidity in conditions that tend to recur over time.
  10. Evaluate patient care plans with primary care team.
  11. Teach patients, families, and care staff prevention and treatment enhancement techniques through psychoeducation and other strategies.
  12. Work in collaboration with patients, families, and providers to develop effective behavioral change strategies to address current behavioral health issues and maximize health.
  13. Complete paperwork as part of patient care in an efficient and timely manner.
  14. Assist with warm hand-offs between clients and organization/provider that BHC is referring to, i.e., specialty behavioral health.
  15. Complete care management phone calls per protocol.
  16. Attend and participate in meetings and Quality Improvement activities as required. Respond to patient or co-worker complaints and work toward a positive resolution of any dispute.
  17. Monitor the site’s behavioral health program, identifying problems related to patient services and making recommendations for improvement.
  18. Employees must work effectively on a team and communicate respectfully with patients and all staff based on shared goals and mutual respect.  This includes demonstrating adaptability to continuous efforts to improve patient care and willingness to be an active care team member engaged in ongoing process and quality improvement projects within the framework of the Patient-Centered Medical Home.

  20. Participate in the development of BHC Student Programming.
  21. Attend department meetings.
  22. Participate in program development activities as supervised by staff members.
  23. Participate in evaluation of peers and support staff.
  24. Serve as a member of site committees as requested and approved by Supervisor.


Qualifications: 
 
  1. Degree and license in Clinical Psychology or Social Work (LCSW).
  2. Current (appropriate State) licensure as a Licensed Clinical Social Worker or Licensed Clinical Psychologist, if not intern status. 
  3. Must have an excellent working knowledge of behavioral medicine and evidence-based treatments for medical and mental health conditions.
  4. Ability to make quick and accurate clinical assessments of mental and behavioral conditions.
  5. High level of comfort in working with primary care medical providers using a consultative style.
  6. Ability to work through brief patient contacts, including via telephone.
  7. Good knowledge of psycho-pharmacology.
  8. Ability to design and implement clinical pathways and protocols for treatment of selected mental and behavioral conditions.
  9. Ability to train and teach effective behavioral health interventions and the primary behavioral health model to medical practitioners and other behavioral health providers.
  10. Ability to work effectively as a team, interfacing with patients, primary care providers, consulting psychiatrist and mental health specialists, as well as with administrative and support staff.
  11. Ability to exercise balanced judgment in evaluating situations and making decisions, and to handle difficult or confrontational situations in a calm, consistent, and equitable manner.
  12. Ability to read, analyze and interpret general business periodicals, professional journals, technical procedures, or governmental regulations.
  13. Ability to multi-task in a fast-paced environment.
