Join our team who is committed to the delivery of the highest quality health care service. We are seeking a full-time Vaccine Outreach Specialist that is self-motivated, energetic, and a take charge individual.
JOB RESPONSIBILITIES:
Utilize the Electronic Health Record (EHR) to connect with current patients and provide preventive health education on the COVID-19 vaccine, in accordance with emerging CDC recommendations.
Based on need, include health education on other preventive health services, such as regular screenings and chronic condition maintenance.
Assist patients with scheduling and confirming appointments, prioritizing the COVID-19 vaccine but including other preventive services, as well as assisting to overcome obstacles to care as needed.
Attain familiarity with other services offered at the clinic to be able to refer patients to other departments for continuity of care.
Develop a workflow with Population Health team and provider teams to coordinate high quality care and access to services for patients.
Maintain a detailed, accurate and comprehensive record of all calls and appointments.
Participate in regular team, department, and organizational meetings, where applicable.
QUALIFICATIONS:
Bilingual English/Spanish required.
Medical Assistant experience or other health care experience preferred.
Excellent oral and written communication skills.
Computer and typing skills.
Ability to relate to people from a variety of backgrounds.
Motivational interviewing experience preferred but not required.
