Job Description
Join our team who is committed to the delivery of the highest quality health care service.  We are seeking a full-time Lead Outreach Worker that is self-motivated, energetic, and a take charge individual.
 
Job Responsibilities:

 
  1. Provide lead poisoning outreach to the community, including home assessment for lead risk; family education regarding lead poisoning risks and how to prevent exposure; finger-stick blood lead tests for children under age six and door-to-door canvassing to homes in the community to identify young children who have not been tested for lead poisoning.
  2. Provide education about lead abatement work and lead poisoning safety during renovation.
  3. Provide referral information and support to families regarding social services and community programs.
  4. Participate in environmental health community education programs, e.g., health fairs, education to childcare programs, etc.
  5. Participate in data collection, record-keeping and data entry for assigned programs, may run routine reports as needed.
  6. Assume responsibility for specifically assigned office activities relative to Lead Project needs.
  7. Assist with other environmental health grant initiatives as needed.
  8. Perform related work as assigned by the Manager of Environmental Health & Community Wellness
  9. Enter data related to community outreach activity in program database and utilize various computer programs to access client data.
  10. Pull and prepare charts for home visit activity.
  11. Maintain general office organization.

Qualifications:
  1. High school diploma or GED.
  2. Experience in community-based agency, community health organization or related field.
  3. Ability to relate well to people of various ages and from diverse ethnic and cultural backgrounds.
  4. Bilingual communication skills required (Spanish/English).
  5. Basic computer skills.
  6. Experience in data entry using Excel, Access or equivalent computer background.
  7. Excellent attention to detail and organizational skills.
  8. Good oral and written communication skills in both languages.
  9. Ability to work occasional evening and weekend hours.
  10. Valid Wisconsin driver’s license, use of an automobile and current auto insurance.
  11. Ability to obtain SSCHC’s internal certification for to the administration of finger stick blood lead tests.
