Job Description
Join our team who is committed to the delivery of the highest quality health care service.  We are seeking a full-time 340B Program Coordinator that is self-motivated, energetic, and a take charge individual.
 
Job Responsibilities:
 
  1. Serves as a medication expert to support SSCHC providers and provide care to SSCHC patients.
  2. Identifies and implements clinical pharmacy services that will be a value add to clinic staff and patients.
  3. Engages with SSCHC clinical leadership and participates in decision-making processes related to the implementation of new clinical processes related to medication therapy.
  4. Engages with SSCHC clinical leadership to ensure appropriate use of clinic administered medications that meets Joint Commission standards.
  5. Oversee SSCHC Clinical Pharmacy Residents in conjunction with Schools of Pharmacy.
  6. Oversee SSCHC Clinical Pharmacy staff.
  7. Assists in updating SSCHC 340B Formulary, educate clinic staff of formulary medications, and assists in review of formulary exemptions.
  8. Conducts Annual Wellness Visits for eligible patients.
  9. Provides direct patient care to patients referred to clinical pharmacy program.

Specific Responsibilities:
  1. Provide direct oversight of growing SSCHC clinical pharmacy team.
  2. Work with clinic leadership to ensure successful integration of clinical pharmacists within our care teams.
  3. Provide vision for expanded roles of clinical pharmacy within SSCHC.
  4. Recruitment of highest quality clinical pharmacist candidates.
  5. Development training programs and clinical pharmacist development.
  6. Oversee relationships with Schools of Pharmacy to recruit and oversee clinical pharmacy fellows, residents, and students to ensure strong recruitment pipeline.
  7. Oversee expansion and updates of collaborative practice agreements.
  8. Oversight of medication use within the clinic in compliance with Joint Commission standards.
  9. Development of medication safety event program.
  10. Assessment of population health data within clinic to provide targeted interventions to optimize therapy (statin use, albuterol use as examples).
  11. Provide clinical pharmacy services to patients, emphasizing health promotion and disease prevention as well as management of acute and chronic health conditions.
  12. Provide medication education, health promotion, and disease prevention to patients.
  13. Recommend or adjust medication prescriptions in accordance with a collaborative practice agreement.
  14. Provides drug information and patient care consults to other health care professionals upon request.
  15. Participates in SSCHC’s Quality Improvement initiatives.
  16. Assists in the development of educational materials for patients and SSCHC providers.
  17. Participates in the creation of ambulatory policies and procedures as requested.

Qualifications:
  1. Licensed as a pharmacist in the State of Wisconsin, with license in good standing.
  2. Insurable for liability coverage as a pharmacist.
  3. Proven high level written and oral communication skills.
  4. Self-motivated and independent, able to work without direct supervision.
  5. Ability to process large amounts of data.
  6. Ability to work well within a team.
  7. Minimum 5 years’ experience as a clinical pharmacist or in clinical pharmacy leadership.
  8. Previous experience acting as site coordinator for a clinical pharmacy residency program.
  9. Ability to relate well to people from diverse ethnic and cultural backgrounds.
