Center Point DAAC is currently recruiting for a full time Outreach Counselor

Qualifications:

• Alcohol and Drug Certification or Registration (California State Approved)
• Knowledge of drug and alcohol addiction, and the effects of the associated stigma, trauma, STD’s, HIV/AIDS, and other collateral issues
• Two years of experience working in a substance use disorder treatment program
• Professionalism, punctuality, flexibility and reliability
• Integrity to handle sensitive information in a confidential manner
• Good verbal, written and interpersonal skills

Job Responsibilities:

• Conduct educational and outreach presentations and activities with agencies, community substance use disorder providers, medical and mental health providers and potential participants
• Provide educational information about medication assisted treatment and available program service and treatment opportunities
• Screen potential participants in jails, prisons, social service and mental health providers and those participating in substance abuse treatment programs
• Assist clients in the development of needs and service plans
• Complete accurate charting and timely documentation and progress notes
• Coordinate all referral services awith the coordinator and other team members
• Assist client’s access to community resources including primary care, mental health, social services, housing, and vocational services
• Assist with follow-up activities and makes referrals for continuing care in the community
• Other duties as assigned
