Job Description

Center Point DAAC is currently recruiting for a full time Client Support Specialist

Qualifications:

  • Alcohol and Drug Certification or Registration (California State Approved)
  • Must have knowledge of community resources and the ability to navigate through governmental benefits enrollment and other community support systems
  • Must have data entry experience
  • Must have California Driver’s License with proof of insurance
  • Professionalism, punctuality, flexibility and reliability
  • Integrity to handle sensitive information in a confidential manner
  • Good verbal, written and interpersonal skills

Job Responsibilities:

  • Conducts case management activities and services with the multi-disciplinary team
  • Provides information and referrals to community partners and other treatment providers
  • Participate in the coordination of referrals to mental health, primary care, and social services
  • Provide orientation to medication assisted treatment program goals and expectations
  • Ensures that basic needs are met, including clothing, hygiene products, navigation assistance for insurance and other benefits and obtaining identification documents, etc
  • Advocates for and acts as liaison to other community based providers and resources and other benefits, housing, vocation training, and employment opportunities
  • Tracks program admissions and discharges
  • Reviews participant data for Quality Assurance
  • Other duties as assigned
