Job Description
Join our team who is committed to the delivery of the highest quality health care service. We are seeking a Physical Therapist that is self-motivated, energetic, and a take charge individual.

Responsibilities:

 
  1. Organize and conduct physical therapy programming for SSCHC to facilitate rehabilitation which includes the adaptation of programming or plan of care according to the needs of the individual patient.
  2. Direct patient participation in selected tasks to restore, reinforce, and enhance performance.
  3. Facilitate the patients’ learning of skills and functions that are essential for adaptation and productivity, to diminish or correct pathology.
  4. Design and deliver effective and efficient rehab services which includes but is not limited to establishing an appropriate plan of care for each patient and documentation of all services provided.
  5. Make appropriate referrals to other disciplines or organizations to address the comprehensive needs of the patient.
  6. Consult with other members of the rehabilitation team to select the most appropriate therapeutic regimen consistent with the needs and capabilities of each patient.
  7. Screen, examine, and evaluate patients which will include but is not limited to taking a medical history, conducting a review of systems, applying appropriate tests and measures, synthesizing information and interpreting examination findings to establish a diagnosis, identifying impairments, determining the predicted level of improvement and the time required to achieve it, and identifying precautions or contraindications to design an appropriate plan of care.
  8. Develop appropriate treatment goals and methods in collaboration with the patient and caregivers, implement the physical therapy treatment plan, and complete all related documentation and record keeping.
  9. Submit billing to coding department within the confines outlined in SSCHC policies.
  10. Designs, fits, and applies adaptive devices and trains patients in the use of adaptive devices.
  11. Instructs patient in the skills and techniques required for participation in functional activities and evaluates patient progress, attitudes, and behavior as related to rehabilitative potential.
  12. Performs routine reexamination as needed to modify progress plan of treatment.
  13. Coordinate P.T. services with medical department.
  14. As necessary, provide supervision to the SSCHC Physical Therapy Assistants in accordance with state licensure requirements and professional standards.

Qualifications:
  1. Licensed to practice as Physical Therapist in State of Wisconsin.
  2. Must have a Master’s degree in Physical Therapy or must have a Bachelor’s degree in Physical Therapy plus 5 years of progressively responsible PT experience.
  3. Ability to communicate effectively and educate patients.
  4. Ability to establish and maintain effective working relationships with coworkers, officials, the public, patients, and other private and public organizational representative.
  5. Have good organizational and time management skills.
  6. Have good verbal and written communication skills.
  7. Must be able to work in a clinic environment that allows for whatever flexibility is needed to meet the immediate patient needs which will include providing care in the medical exam room.
  8. Ability to work under pressure, maintain personal composure, evaluate situations and act/react appropriately, and use tact in dealing with visitors, patients, and difficult situations.
  9. BLS Health Care Provider Certification.
  10. Experience in Public Health and with multi-cultural populations preferred.
  11. Bilingual (Spanish/English) preferred.
