Job Description
We are seeking a full-time Prevention Navigator that is self-motivated, energetic, and a take charge individual.
 
Job Responsibilities:
 
  1. Coordinate preparation of and conduct outreach activities to engage individuals and communities disproportionately impacted by health issues related to HIV, STIs, SUD, and HCV. Outreach activities may be performed during or outside of office hours and include, but are not limited to, community events, bar outreach and the use of social media platforms.
  2. In compliance with the WI DHS Counseling, Testing and Referrals (CTR) Protocol, conduct confidential and anonymous CTR for HIV, STI and Hepatitis C Virus (HCV) focusing on individuals at high risk for infection.
  3. Assist clients of our syringe exchange program to access clean equipment for safe injection and overdose prevention.
  4. Assist clients to navigate internal and external systems and processes to access appropriate care and treatment for HIV, Pre-Exposure Prophylaxis (PrEP), Post Exposure Prophylaxis (PEP), HCV and Substance Use Disorders (SUD). Navigation services include but are not limited to facilitation of access to medical care, behavioral and mental health services, case management and/or social services, pharmacy services, health coverage and Medication Assistance Programs.
  5. Monitor outcomes of referrals and assist in evaluating patterns of barriers that clients may face when attempting to access resources and work toward resolution as appropriate.
  6. Conduct individual and/or group level educational sessions on the subjects of sexual health and SUD, including prevention and care of HIV, STIs, HCV, harm reduction and overdose prevention.
  7. Maintain files for each client participating in prevention interventions in compliance with State and Sixteenth Street protocols and complete required data entry in the appropriate platforms.
  8. Develop promotional and educational materials related to the services provided at the Prevention Center.
  9. In coordination with Prevention Manager, connect with community partners, businesses, other stakeholders, and residents to develop partnerships and identify/create opportunities for collaborations, increase awareness and acceptance of HIV prevention strategies and related services including CTR, PrEP, syringe exchange, sexual health, risk reduction and harm reduction.
  10. Under the coordination of the HIV Prevention Manager and HIV Director participate in strategic planning process, the preparation of grant applications, and submission of progress reports for all prevention interventions.
  11. Work closely with other members of the prevention team to successfully achieve programmatic goals while maintaining the organization of the Prevention Center to offer an inviting environment to our clients, this includes maintaining supply inventories, procuring the cleanliness of office and work areas, and contributing with ideas for improvement of customer service experience.
Qualifications: 
 
  1. Bachelor’s Degree in social work or human services desired, consideration will be made for a suitable combination of previous academic and professional experience.
  2. Experience working with diverse populations including people at risk for HIV, people living with HIV, people who inject drugs, or individuals who identify as LGBTQ.
  3. Bilingual (Spanish/English) fluency preferred.
  4. Excellent organizational skills
  5. Excellent oral and written communication skills.
  6. Valid Wisconsin driver’s license, use of an automobile and current automobile insurance.
