Job Description

Location: 1920 Libal St, Green Bay, WI 54301

Shift Hours:

Full-Time 1.00 FTE (40 hours/week), Monday through Friday days. No weekends or Holidays

Job Description:

The Community Resource Navigator will work in collaboration with the care teams to support the efforts to achieve population health initiatives. This position works closely with all areas of the Extended Care Team, including providers, patients and families. Works collaboratively with internal and external partners in order to advocate for patient needs, to reduce duplication of effort, and to assure service information is current and accessible. Responsibilities include sharing of pertinent patient information with the team, performing outreach and education to patients and families, assisting with extended care team tasks, and identifying opportunities to enhance care coordination for the population. Scheduling appointments and connecting patients with needed services. Other responsibilities include developing, maintaining, using and disseminating a comprehensive, reliable, and accurate community resource database.

Qualifications:

Associate or bachelor's degree preferred.

Valid Wisconsin or Michigan driver's license required.

Two years recent experience in a health care or human service related field.
