Location: Bellin Memorial Hospital Campus- 744 S Webster Ave, Green Bay WI 54301

Shift Hours:

Full-Time 1.00 FTE (40 hours/week), M-F 08-1630

Program Description:

Hospital to You is Northeast Wisconsin's only approved Acute Hospital Care at Home (AHACH) waiver program through the Centers for Medicare and Medicaid Services. Patients who meet eligibility criteria (clinical, demographic, and safety) are offered the option of continuing their inpatient stay in the comfort of their own homes. Patients receive all of the same services at home as they would receive in the hospital, excluding on-site 24/7 nursing services. Services include daily provider rounding (in-person or video), in-home vital sign monitoring and lab draws; medical equipment and transportation; mobile imaging, medications, and meals. Patients receive care from a robust team including physicians (hospitalists), advanced practice clinicians (APCs), nurses, nursing assistants, social workers, and physical, occupational, speech, and respiratory therapy. Care team access is 24/7 with the ability to respond on-site within 30 minutes. We utilize advanced technology through our remote patient monitoring program to closely monitor patient vital signs and other key clinical information including listening remotely to heart and lung sounds, capturing a 1-lead electrocardiogram, or conducting a video visit

Job Description:

Responsible for managing and facilitation the admission or transfer into the Hospital to You program. The coordinator is responsible for arranging uninterrupted continuing medical care of the patient and assisting in the organization of family resources for the effective care of the patient at home. The coordinator's duties may be performed at the hospital or other health care locations and include:

* Identify eligible candidates

* Obtain provider approval and patient consent

* Coordinate patient care needs with the multidisciplinary team

* Marketing of program to key stakeholders.

* Design and implement growth strategies.

* Collaborate with Hospital at Home programs regionally and nationally to determine best practice.

* Collect program data and auditing to ensure high quality care delivery.

* Routine evaluation of protocols and policies and procedures.

Qualifications:
  • Bachelor of Science in Nursing (BSN) degree preferred.
  • Wisconsin RN license required.
  • Valid Wisconsin driver's license in good standing and liability car insurance required.
  • Minimum of one year's recent experience in a hospital setting
  • Sign on Bonus available for qualified candidates.
