Job Description
Join our team who is committed to the delivery of the highest quality health care service. We are seeking a full-time CCS Care Coordinator that is self-motivated, energetic, and a take charge individual.

General Statement of Duties:
The Comprehensive Community Services (CCS) Care Coordinator is responsible for helping the consumers to understand, navigate, and grow through the CCS program.  This Care Coordinator meets and works with the consumers, service providers, and other team supports that assist in the consumer’s progression through the CCS program.  The Care Coordinator receives consultation from mental health and/or substance abuse professionals while working on the clients individualized Recovery Plan of Care (RPOC)

Responsibilities:
  1. Develop and update Consumer treatment plans, review documentation of ancillary providers, conduct 6 month and annual reviews in the Electronic Health Record. 
  2. Participate in staff training, read materials, program forms, and other program documents.
  3. Engage in ongoing staff training, coaching, skill development, and performance evaluation.
  4. Ensure clients are receiving quality services.
  5. Provide ongoing documentation for services rendered to the consumers/clients. 72-hour guideline for note entry.
  6. Provide coverage for staff, as needed, for vacations and holidays when applicable.

Knowledge, Skills and Abilities:
  1. Proficient in computer applications including Microsoft Office Suite (Word, Excel, PowerPoint) and web enabled databases.
  2. Excellent oral and written communication skills.
  3. Excellent organization and time management skills, attention to detail and accuracy, and ability to prioritize multiple tasks.
  4. Ability to work independently and as part of a team.
  5. Demonstrated ability to work well with individuals from diverse economic, racial, and ethnic backgrounds. 
  6. Knowledge and understanding of mental illness, substance use, and the effects of poverty on various socioeconomic social groups.
  7. Knowledge of community mental health and substance use treatment resources

Qualifications:
  1. Must posses a B.A. or B.S. degree in Social Work, Psychology, Nursing, or a related field with experience in human services and case management, Master's degree preferred. 
  2. Prior experience in leading or managing treatment teams in a human service program preferred.
  3. Valid Wisconsin driver’s license and automobile with insurance required in good standing.
  4. Pre-employment background check
  5. Proficient in the use of the internet, SharePoint, Teams, and email applications.
