Job Description

Location: The Cancer Team: 1580 Commanche Ave, Green Bay, WI 54313

Shift Hours:

0.80 FTE (32 hours/week) Days 8am-4:30pm. No weekends, no holidays.

Job Description:

The Oncology Patient Navigator will work with Oncology patients and their families to assist with understanding their disease and treatment options.

The Oncology Patient Navigator supports patients with cancer, their families, and their caregivers to assist them in overcoming any barriers they may be facing in the healthcare system and facilitate timely and quality access to care through all phases of the cancer journey.

The patient navigator is a primary point of contact for the patient and works with other members of the care team to coordinate care for the patient. This critical person on the multidisciplinary team provides an important perspective on the logistical, structural, and social needs of the patient, as well as cultural considerations, patient values, and care preferences.

They guide the patient through the cancer care continuum from diagnosis through survivorship. More specifically, the oncology patient navigator acts as a central point of contact for a patient and assists in coordinating all components involved in cancer care, including surgical, medical, and radiation oncologists; patient education; community support; financial and insurance assistance; and others. This person has a clinical background and is a critical member of the multidisciplinary cancer team.

Knowledge, Skills & Abilities Required:

*Ability to communicate complex clinical and regulatory concepts in layperson language.

*Ability to build rapport with diverse patients and peers.

*Demonstrated problem-solving, critical decision makings and professional judgment.

*Strong organizational skills and attention to detail.

*Ability to work successfully both independently and in team settings.

Qualifications:

Bachelor's degree in Social Work, Nursing, Psychology, Human Development, and/or related degree required. If RN or Social Worker, license required.

Skills in assessing, planning, and managing patient care in an acute care hospital or other health care facility recommended; background in care management, utilization review and discharge planning is beneficial; knowledge of available community resources to assure the continuum of care is beneficial.
