Description

PRIMARY JOB FUNCTION(S):

  1. Responsible for providing skill training, personal assistance care, support, and constant supervision to a group of people working in the community for a minimum of 20 hours per week.
  2. Completes data collection for each person assigned.
  3. Completes task analysis and time studies in accordance with Federal and State Department of Labor regulations.
  4. Administers medication as prescribed on the Physicians Medication Order Form.
  5. Acts as the Agency representative with employers, volunteer coordinators and the public.
  6. Ensures people served are picked up and dropped off at site by Mass Transit Administration (MTA) mobility or designated mode of transportation.
  7. Implements behavior programs in accordance with approved Behavior Plan.
  8. Handles all emergency situations in accordance with Agency Policies and Procedures.
  9. Performs quality control of site to assure all contract specifications are met.
  10. Works a flexible schedule including nights, weekends and holidays in accordance with the employer=s contract specifications and needs of the persons served.
  11. Must report to assigned community employer location during inclement weather, illness or other absence of the person employed and completes job responsibilities for the person.
  12. May be required to drive an agency vehicle from a Chimes day program to the contract site.
  13. Must follow dress codes as specified by the employer and meets Occupational Safety and Health Administration (OSHA) requirements.
  14. Must have at all times in their possession a working personal cell phone for company use.
  15. Ensures all supplies and equipment needed to perform the contract(s) is present and in proper working order at all times.
  16. Must eat meals with and provide constant supervision to all people assigned.
  17. Presents a positive role model for the persons served and Agency staff.
  18. Uses technology for the completion of specified job duties.
  19. Attends work regularly according to assigned work schedule and in accordance with Agency policy.
  20. Attends and participates in all in-service training, staff meetings and other activities to facilitate professional development.
  21. Works cooperatively with others including all staff, supervisors, administrators, co-workers, people served, community professionals, customers, vendors and the public.
  22. Assumes other duties, responsibilities and special projects as needed.

SECONDARY FUNCTION(S):

  1. May be assigned to transport person served from their residence to a job site.
  2. May be required to drive a personal vehicle in safe working condition to transport the person.
  3. Works cooperatively and effectively with Agency volunteers maximizing the volunteers= experience while meeting the needs of person served and the programs.

REQUIRMENTS:

EDUCATION: High School diploma required; Associate of Arts degree in special education or a related field a plus. Experience working in creating community involvement and volunteer opportunities with persons with developmental disabilities and/or related disabilities preferred.

EXPERIENCE: Must have an acceptable driving record with full driver's license as determined by criteria established by the Agency's insurance carrier and by Agency policy. Must be able to lift a minimum of 50lbs based on the needs of person served.
