JOB SUMMARY:
The program director ensures the mission of Chase Brexton Health Care to integrated high quality of care, while addressing inequities is carried out through care coordination. The program director will be responsible for building teams to carry out HIV care, Diabetes Mellitus Management, and other key programs smoothly while collaborates with operational and functional management to identify improvement opportunities, program expansion, and cost reduction opportunities. Communicates with all team members daily to ensure that deadlines are met, and roles are understood. The program director leads and manages budgeting and reporting aspects of the program and supervise staff as assigned.
MAJOR DUTIES AND RESPONSIBILITIES:
Analytical and Critical Thinking
* Provides oversite of the programs through monitoring, reporting and quality assurance activities
* Provide programmatic, clinical, quality, and operational supervision to all team members
* Monitor progress towards established goals and quality indicators and create action plans for improvement as needed
* Manage administrative and programmatic aspects of the programs, including reporting, budgeting, grant proposals, and liaison to grantor (if applicable)
* Support Ryan White Program by ensuring required changes are incorporated into program and ensure quality metrics are measured and goals are achieved.
* Oversee ongoing quality management activities and integrate quality improvement into daily program
* Oversee the day-to-day functioning of programs
Communication
* Communicate with staff regularly and concisely
* Ensure staff training and compliance is in accordance with policy and procedures
Teamwork
* Collaborate with departmental leads to develop workflows
* Collaborate with departmental leads to identify and implement key data points for programs developments
* Collaborate with health care providers, clinical, operational and support staff to ensure that the full continuum of care is brought to HIV-positive patients, Diabetes Mellitus patients and other programs' patients as assigned
Resource, Staff, and Budgets
* Accountable for program implementation/expansion, financials, and deliverables
* Coordinates delivery of work from all parties involved

* Evaluates risks and recommends mitigation measures

* Produces reports to relevant level of management
* Plans, monitors, and reports on the status of assigned programs
Compliance, Policies and Procedures
* Ensures all policies and procedures are in place and followed accordingly
* Collaborates with IT to ensure adherence to IT standards, policies, and procedures*
SKILLS AND ABILITIES:
* Demonstrates advanced knowledge regarding the use of community resources and strategies to support under-resourced patients.
* Proven ability to address the social determinants of health and barriers to healthcare access and engagement.
* Extensive experience working with marginalized populations who may have social, economic, and/or behavioral health issues.
* Experience facilitating social/clinical support groups.
* Experience educating healthcare professionals and patients on chronic disease mitigation strategies.
* Working knowledge of Lean or Six Sigma methodologies, preferred but not required.
* Program evaluation and management experience
* Ability to participate in and facilitate discussions with complex content is required.
* Must possess excellent organization, project, program management and problem-solving skills
* Interpersonal skills and the ability to communicate concisely and positively to all team members
* Proficiency with Microsoft Office Word, Advance Excel, Power Point Presentation
EDUCATION AND/OR EXPERIENCE:
* Master of Business administration and / or Master of Public Health
* Licensed Clinical Social Worker (LCSW) desired
* Bachelor's degree in health-related field.
* Minimum of 5 years' experience in health care setting managing similar services
* Strong understanding of cultural competency with targeted population
