Job Description
Join our team who is committed to the delivery of the highest quality health care service. We are seeking a full-time Behavioral Health Consultant that is self-motivated, energetic, and a take charge individual. 

Responsibilities: 
  1. Support Primary Care Providers (PCP) and improve the effectiveness of the health care provided by PCPs by using a team approach and addressing the entire range of the patient’s presenting problems.
  2. Consult with patients in brief appointments (typically 30 minutes or less) and assist the PCP with assessment and co-management of target problems.
  3. Conduct assessments and brief, focused interventions, gearing their services toward early identification, triage, quick resolution, and long-term wellness.
  4. Encourage PCPs to refer any patient who may have biopsychosocial or behavioral difficulties, or factors affecting their health or functioning.
  5. Provide timely and succinct feedback to PCPs about all referred patients.
  6. Address a wide range of health and behavioral health complaints and engage in health promotion.
  7. When not in the primary care clinic, BHCs are readily available to the PCP by phone or pager during regular office hours.
  8. Provide phone consultation, when necessary, even if it means accepting interruptions.
  9. Document findings and recommendations in the client’s integrated record.
 
If also working in the traditional Behavioral Health model responsibilities also include:
 
  1. Promote healthy personal and family relationships through education, exploration of relationship dynamics, and therapeutic intervention.
  2. Conduct weekly therapy sessions with clients; additional sessions are provided on as “as needed” basis
  3. Provide each client with a custom-designed treatment plan which will include measurable, realistic and appropriate treatment goals, and outline specific therapeutic interventions utilized by the psychotherapist, ongoing assessment of progress, modifications when necessary, and aftercare planning.
  4. Continuously review client treatment plan and record therapy sessions with progress notes. Insure notes contain accurate and relevant information and stay within legal parameters.
  5. Consult with a psychiatrist or psychologist after initial contact with a client and as needed, with client treatment issues as they come up.
  6. Collaborate with referents and community resources and make requested court appearances as they pertain to the client’s continuity of care.
  7. Attend clinical case reviews, psychiatric consultations, medical reviews, in-services and workshops that maintain and improve clinical skills.
 
Qualifications: 
 
  1. Possess a master’s degree and/or PhD with current license.
  2. Successful completion of the SSCHC in-house BHC training/orientation program.
  3. Must be a friendly, dynamic communicator with the ability to multi-task and make sound decisions quickly especially in an environment that produces a high level of stress.
  4. Must have good leadership skills.
  5. Must be assertive.
  6. Must speak in a clear voice and have a pleasant attitude.
  7. Ability to relate well to persons from diverse ethnic and cultural backgrounds.
  8. Must be bilingual (Spanish/English OR Hmong/English).
