Job Description
Join our team who is committed to the delivery of the highest quality health care service. We are seeking a full-time Vaccine Outreach Specialist that is self-motivated, energetic, and a take charge individual.
JOB RESPONSIBILITIES:

JOB RESPONSIBILITIES:
- Utilize the Electronic Health Record (EHR) to connect with current patients and provide preventive health education on the COVID-19 vaccine, in accordance with emerging CDC recommendations.
- Based on need, include health education on other preventive health services, such as regular screenings and chronic condition maintenance.
- Assist patients with scheduling and confirming appointments, prioritizing the COVID-19 vaccine but including other preventive services, as well as assisting to overcome obstacles to care as needed.
- Attain familiarity with other services offered at the clinic to be able to refer patients to other departments for continuity of care.
- Develop a workflow with Population Health team and provider teams to coordinate high quality care and access to services for patients.
- Maintain a detailed, accurate and comprehensive record of all calls and appointments.
- Participate in regular team, department, and organizational meetings, where applicable.
QUALIFICATIONS:
- Bilingual English/Spanish required.
- Medical Assistant experience or other health care experience preferred.
- Excellent oral and written communication skills.
- Computer and typing skills.
- Ability to relate to people from a variety of backgrounds.
- Motivational interviewing experience preferred but not required.
- Knowledge of SSCHC and the community it serves.
