Job Description

Job Summary:
The Community & Day Specialist is responsible for supporting individuals with intellectual and developmental disabilities as they participate in meaningful community activities and helping them obtain volunteer opportunities and paid jobs. This position will identify opportunities for individuals with disabilities in the community to increase their employment/activity skills, habits, and behaviors to achieve their goals and lead to greater community involvement and an enhanced quality of life.

Duties and Responsibilities:

Day Supports (85%):
•    Administer primary health care or medical treatments
•    Encourage and teach self-advocacy skills
•    Provide support to find employment and meaningful community activities
•    Provide counsel, comfort, or encouragement to individuals or families
•    Drive vehicles to transport patrons
•    Acts as an advocate for individuals when they cannot do for themselves
•    Assist in the development of friendships and other relationships
•    Assist individuals in becoming active participants in their community and build natural support networks
•    Maintain comfort, privacy, and dignity of the individual through the delivery of services 
•    Care for patients by changing bed linens, washing and ironing laundry, cleaning, or assisting with their care.
•    Administer prescribed oral medications under the written direction of a physician or as directed by a home care nurse or aide, and ensure patients take their medicine.

Administration (15%):
•    Documents case notes
•    Maintain client information or service records.
•    Report special incidents and suspected abuse according to applicable laws and regulations
•    Attends staff and other meetings as necessary
•    Prepare and review documentation related to Personal Care Plan 
•    Participate in case of reviews, consulting with the team caring for the client, evaluating the client's needs, and planning for continuing services. 
•    Other duties as assigned by the supervisor

Required Skills/Abilities: 
•    Must possess a strong service orientation, including active listening, critical thinking skills, and close monitoring of clients.
•    Comfortable in an environment where patients/clients cannot perform essential life duties.
•    Understanding primary personal care, housekeeping, and other care areas.


Required Qualifications:
•    High School Diploma or GED equivalent 
•    Valid Driver’s License 
•    Proof of Vaccination

Preferred Qualifications:
•    Minimum 1 year working with individuals with intellectual and developmental disabilities 

Physical Requirements: 
•    Prolonged standing, leaning, walking, bending, and lifting periods.
•    Must be able to lift to 50 pounds at times.
