Job Description
Join our team who is committed to the delivery of the highest quality health care service. We are seeking a full-time Ryan White Housing Program Coordinator that is self-motivated, energetic, and a take charge individual.

The Team: 

The Sixteenth Street Community Health Center’s Lead Outreach Program (LOP) is the renowned, cutting edge educational and outreach arm of our clinic’s fight to end childhood lead poisoning on the south side of Milwaukee. In the last 25 years we’ve helped to decrease the incidence of childhood lead poisoning dramatically, but there is still much work that needs to be done. Our community health workers work directly with families with children five years old and younger to help mitigate lead poisoning. We achieve this by developing relationships with each family, providing water filters and other necessary supplies, and coordinating with medical professionals. in our own clinic, at other clinics, and at the Milwaukee Health Department. We provide high quality and comprehensive care for the families enrolled in our program. We also have developed unique programs to do educational outreach and lead testing in the community, work with pregnant women and mothers of infants to prevent childhood lead poisoning, and connect families to the Wisconsin Lead Safe Homes Program to help remove lead contaminants from the home and make the property safe for generations to come.

The Role:

Provides administrative support to all aspects of the Lead Outreach Team including coordinating appointments, processing incoming referrals, communicating with internal and external stakeholders, managing databases, and providing training and mentorship to LOP staff.

Job Responsibilities:
 
  1. Assist the Manager with coordination of program initiatives, including:
  • Mentoring and conducting orientation and training activities, as necessary, for new and existing staff related to LOP procedures and systems.
  • Evaluation of program outputs, and client outcomes for quality improvement.
  1. Serve as primary liaison to SSCHC medical providers, the Milwaukee Health Department, Dynacare Laboratories, and other clinical staff to ensure appropriate coordination of lead testing, education, and follow-ups for current and prospective LOP participants.
  2. Serve as primary liaison to EPIC Site Specialists to ensure proper database protocols are adhered to, and train and assist staff with questions as an EPIC Super User.
  3. Schedule home visits and appointments to families of children identified as high risk for lead poisoning.
  4. Process referrals for families of children with elevated blood lead levels or at risk of lead poisoning to the LOP and disseminate these referrals to appropriate LOP staff.
  5. Collect and enter data into multiple databases and maintain records associated with these databases. Assist in compiling data for monthly, quarterly, and annual reports.
  6. Assist staff with providing basic childhood lead poisoning prevention education to clients as needed.
  7. Assist with weekly door-to-door canvassing on Milwaukee’s south side, between April and October, to offer LOP services to non-patient residents.
  8. Assist staff with home-based and community outreach (e.g. schools, churches, community groups) and education related to childhood lead poisoning prevention, including printing and collating educational materials for use in community outreach activities as needed.
  9. Order and track the inventory of office and medical supplies for the team; prepare lead testing kits for use.
Qualifications:
 
  1. Effective bilingual written and oral communication skills (Spanish/English).
  2. High school diploma or GED; Bachelor’s Degree preferred.
  3. Previous administrative experience required; experience working in a health care setting preferred.
  4. Familiarity with database management and administration preferred.
  5. Excellent attention to detail, strong organizational skills and ability to meet deadlines.
  6. Proficiency with data entry and MS Office applications.
  7. Ability to work well in a team environment with minimal supervision.
  8. Ability to relate well to people from diverse ethnic and cultural backgrounds.
  9. Ability to work occasional evening and weekend hours.
  10. Valid Wisconsin driver’s license, use of an automobile and current auto insurance for attending meetings and other functions that take place outside of the office.
  11. Ability to obtain SSCHC’s internal certification for to the administration of finger stick blood lead tests.
