Job Responsibilities:
Assist in smooth work flow of clinic by:
Anticipating the clinical needs patient and team though a solid knowledge base;
Performing CLIA waived, or other point-of-contact testing, including but not limited to: peak flows, UCG, Urinalysis, Q-streps, HgA1C, culture collections, Hgb, INR, Glucose level, spirometry
Assist in facilitating patients between Medical and AODA/MAT Assisting patient with “Exiting”- “Plan of Care” needs, including but not limited to educational reinforcement of medications, appointment/referral assistance and case-specific education, in a manner understandable by patient
Empower patients through advocacy, education related to chronic conditions, rights and responsibilities, involving patient in plan-of-care decisions, addressing clinic/care related concerns, and involving family in plan of care to the extent allowed by patient
Maintain cleanliness of work area
AODA and MAT case coordination and follow up:
Maintain documentation associated with Medicated Assisted Treatment program
Give injection for Vivitrol
Participate in weekly Medicated Assisted Treatment meetings
Perform initial nursing assessments for MAT patients, including ordering standing lab orders
Provide support and outreach to MAT patients through the recovery process
Be a liaison between AODA providers and Medical providers to ensure continuity of care for MAT patients
Perform related CLIA waived collection and testing, as needed
Provide guidance to Medical Assistants, including provide correction and direction to support staff as needed, participating in testing and in-servicing and acting as a resource for questions/concerns.
Assist in maintenance/protection of patient privacy and medical record data collection by proactively upholding clinic guidelines and standards related to Patient Confidentiality.
Participate in the Clinic mission to provide high quality healthcare by:
Actively participating in department In-servicing and Annual Skills Testing
Actively seek independent studies so as to keep aware and current of nursing issues
Alerting appropriate staff and/or acting on possible safety or confidentiality issues
Offering workflow, safety, morale related improvement suggestions
Perform clinical interventions (injections, catheterizations, IV insertions, phlebotomy) as needed.
Comply with SSCHC Infection Control Plan including employee testing.
Provide walk-in and triage patient services as needed.
Qualifications:
1. Registered Nurse with current license and two years nursing experience; current basic life support certification (CPR).
Supervisory experience or demonstrated supervisory aptitude.
Strong computer skills preferred.
Experience in community health, public health or in a community agency.
Ability to relate well to people from diverse ethnic and cultural backgrounds.
Bilingual preferred (Spanish/English or Hmong/English).
