Clinical Pharmacy Specialist-Full Time
Oaklawn Hospital
Marshall, Michigan

Hours: This position works primarily Monday through Friday day shifts, with occasional weekends as needed.

As a full time, exempt Clinical Pharmacy Specialist with CPS, at Oaklawn Hospital, you will provide clinical pharmacy services for patients who are referred by a provider to a medication therapy center specialty clinic or as part of team-based care embedded within a primary care practice with a focus on Diabetes.

Duties include:

  • Performing direct patient care under collaborative practice agreements and consultation services.
  • Actively managing medication therapy, complete medication reviews, collaborate with providers to optimize medication therapy, improve adherence, promote wellness, prevent disease and complications from disease, and improve outcomes. (This may include recommending or initiating new medication, titrating current medications, monitoring labs, and providing patient education to optimize medication adherence and lifestyle choices).
  • Documenting all activities and interventions accurately and appropriately in patient medical record and communicate with other healthcare providers regarding the patient's care.
  • Collaborating with providers, patients, and caregivers to provide coordinated, comprehensive, high-quality care to patients, including but not limited to disease state management, preventive care, and transition care management.
  • Providing patient education, promote self-management skills, and encourage patient engagement in their own care.

About the Facility: Oaklawn Hospital founded more than eight decades ago is a highly regarded independently owned regional health care organization, licensed for 77 acute care beds, offers over 35 specialties and operates a 17-bed inpatient psychiatric unit. Oaklawn Hospital is a not-for-profit hospital, with a main campus residing on the same site as the original hospital, providing facilities, equipment and technology that are usually only found at larger health systems. Oaklawn has a reputation for advancing medicine and providing compassionate, personal care to Calhoun County and parts of Branch and Eaton counties with a medical staff of more than 150 providers representing over 35 specialties. CPS has partnered with the hospital to manage the day to day operations of this pharmacy. This position is an employee of CPS.

Required Qualifications:

  • B.S. or PharmD from an ACPE-accredited School of Pharmacy
  • Active Michigan Pharmacist license in good standing
  • At least 2 years of recent Pharmacist experience (in a hospital setting preferred)
  • Successful completion of a Ambulatory Care Pharmacy Practice Residency preferred
  • Certification as a Diabetic Educator or ability to obtain is required
  • Ability to work independently, set priorities, make critical decisions and respond to client concerns
  • Experience using computer applications including word processing, spreadsheets and pharmacy information systems
  • Effective communications skills; verbal and written

Hospital Requirements:

  • PPD/TB Skin Test- documentation of negative TB skin test within the last 12 months. Skin test provided on-site, if needed.
  • (Flu) Influenza-required for hire between Oct 1st-April 30th, vaccine provided on-site if needed
