Transition coordinators (TCs) are responsible for acting as a conduit as well as providing services to transition-age youth (14-21) with education disabilities. TCs take a proactive role within the school, including IEP participation and provision of informational seminars focused upon family and student transition needs. Seminars address postsecondary education and/or vocational interests. Transition coordinator contribute to transition plans and assist with goal setting for students. TCs are the direct liaison between the School-to-Work Program and schools and families.
Transtion Coordinators are available to respond to student issues identified at community worksites. Community Assessment Training program (ATP) teachers and TCs work to support students in regard to assessment, behavior, personal circumstance, etc. Transition coordinators are centrally assigned to the School-to-Work Transition Program.
