SUMMARY: Evaluates treats and rehabilitates patients, students or clients by providing them with necessary therapeutic intervention. Maintains pertinent documentation, carries out necessary therapeutic intervention. Maintains pertinent documentation, carries out appropriate treatment plans, and contributes to discharge planning.
ESSENTIAL DUTIES AND RESPONSIBILITIES include the following ( other duties may be assigned)
Completes screens (as scheduled)
Evaluation and accompanying documentation are completed at the time of the evaluation.
Obtains and verifies physician orders prior to initiation of evaluation
Payer source is identified and secured prior to initiation of evaluation
Identifies objective findings and problems
Establishes appropriate short/long term goals
TREATS PATIENTS:
Documents progress in attainment of goals on a timely basis
Utilizes current body of knowledge, skills, and techniques
Interacts with patients/families in an appropriate, safe and professional manner
Designs adaptive equipment and orthotics
Designs and/or implements home programs (as appropriate)
Communicates appropriate referrals to team member and/or supervisor
Assists in development and implementation of new programs/techniques to enhance therapy services (as assigned)
Balances and maintains an appropriate caseload to meet company and facility productivity standards Initiates and follows through with restorative programs as appropriate
Attends patient care meetings and actively participates (as assigned)
Follows through with discharge planning including: equipment ordering, family teaching, home safety recommendations, and discharge documentation (as appropriate)
DOCUMENTATION/PAPERWORK:
Daily documentation is accurately completed each day
Billing logs are accurately completed on a daily basis
Weekly notes are completed as required per specific setting
Time sheets are accurately completed and submitted on time to manager
Documentation accurately reflects treatment program, progress and other factors affecting treatment
Maintains QI documents accurately (as assigned)
COMMUNICATION AND TEAMWORK:
Maintains consistent communication with supervisor and team
Interacts all staff in a professional and courteous manner
Coordinates patient care with other disciplines
Responds positively to constructive feedback and seeks clarification as needed
Flexible and willing to assume additional responsibilities when the need arises
Listens carefully to directions/problems and responds appropriately
When in doubt, asks for clarification and feedback when needed
Displays good organizational and time management skills; ensures coordinated service delivered to patients and families
Proactive in problem identification and seeks appropriate guidance from supervisor as needed
STUDENT AFFILIATION (as assigned):
Serves as clinical instructor for student
Supervises student daily
Co-signs documents daily
Evaluates student performances at midterm and final
PROFESSIONAL STANDARDS:
Adheres to facility dress codes
Maintains patient and staff confidentiality
Adheres to established facility policy and procedures related to environmental safety, orderliness, risk management, infection control, universal precautions
Adheres to company policy and procedures
Adheres to company policy concerning tardiness, absenteeism, and call in procedures
Follows appropriate procedure for scheduling time off including sufficient coverage information on each patient
Attends company meetings as required
Maintains work area in safe, neat orderly fashion (as applicable)
Willingly accepts new duties as assigned
Flexible to travel to all assigned facilities daily
CONTINUING EDUCATION:
Attends facility related in services as appropriate to facility
Provides in-services to facility staff as needed
Sets a positive example for all employees and shares knowledge with new/less experienced clinicians
Maintains awareness and understanding of reimbursement issues as related to state, facility and company guidelines/laws
Attends conferences/seminars
QUALIFICATIONS

Education and/or Experience
Master’s degree (M.S) from four-year college or university; related experience and or training; or equivalent combination and experience
Language Skills
Ability to read and interpret documents such as patient medical record, physician orders, and procedure manuals. Ability to write routine reports and correspondence. Ability to speak effectively before small groups or facility staff family members or fellow employees.
Mathematical Skills
Ability to add, subtract, multiply and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percentages.
Reasoning ability
Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Ability to interpret a variety of instruction furnished in written, oral, diagram, or schedule form.
Certificates, Licenses, Registrations
License to practice allied health profession in Massachusetts or eligible for license, valid Massachusetts driver’s license, proof of CPR certification or eligible for certification.
