Job Description

Center Point DAAC is currently recruiting for a full time Transitional Counselor.<BR><BR>

Qualifications:
  • Experience in small group dynamics and structured environments
  • Demonstrated ability to supervis others
  • Ability to conduct and participate in group and family counseling sessions
  • Ability to relate and work with other treatment and criminal justice personnel
  • Bachelor's degree in a social science is preferred.
  • Must possess knowledge of alcohol and other drug addiction, drug abuse patterns and the homeless
Job Responsibilities:
  • Intake assessment, screening
  • Makes referrals for participants in accordance with service plan
  • Conducts client outreach activities
  • Provides information and referral for permanent housing
  • Refer participants to appropriate community based services
  • Conducts follow-up and other client tracking activities
